We emphasize early intervention for medical problems and utilize testing, specialty care, and old fashioned listening to solve complex problems for patients who need care.

Bastian Fagginger-Auer, APRN, FNP-C

Bastian has been providing patient care in the Rutland area for 25 years . He started his healthcare career in EMS, serving with volunteer and professional services, before transitioning into nursing where he gained experience and knowledge in Emergency Medicine, Surgical Services, Education, and Leadership. As a life-long student, he Bastian has earned degrees from Castleton University, Walden University, and Norwich University. He is certified in Family Medicine through the American Association of Nurse Practitioners.

Bastian was born and raised in Wallingford, Vermont. Aside from living in North Carolina for a few years after college, he has been a continuous resident of Rutland County, which affords him insight into the local area and the complexities that accompany living in Vermont.

At True Care Vermont, Bastian hopes to leverage his expertise and experience to provide holistic care to patients and families.  With his extensive knowledge of the local medical system, Bastian is excited to assist patients in managing their health and wellness using preventative approaches, as well as acute medicine.

Bastian lives at home with his wife, two teenage sons and their four Cavalier King Charles Spaniels.  He has coached athletics at all levels but is enjoying his time as a bystander and fan.  His hobbies are wide and varied as he is a tinker and loves to discover how things work.

Tom Ensalata

Tom Ensalata - Physician Assistant

Tom graduated from Yale University in 2023 with a Master of Medical Science. Prior to attending PA school, Tom worked as a mental health technician for ten years in a psychiatric hospital diversion program. There he served the complex needs of patients with major mental illness and/or mental health crisis. Tom also worked as a developmental services case manager working with clients with severe autism. In addition Tom has 7 years of experience working as a volunteer dementia caregiver. Tom resides in Addison County Vermont with his two dogs and his partner. Tom enjoys spending time with his partner and dogs, as well as cooking, reading, science communication, animal care, and hiking.
